New service aims to enhance patient communication

 

Summit Medical Group has partnered with HealthGrid to roll out a suite of patient engagement features over the next several quarters.

The first service is called Care Summaries. Three days after visiting with your primary care provider, you will receive a text message with a secure link. Click on that link, and use your last name and date of birth to access the summary from your appointment.

The information will include three sections:

  • Visit Summary
  • Health & Medication Education
  • Medical History

 

The Visit Summary will provide your doctor’s notes outlining your assessment and plans for treatment, as applicable. The Health & Medication Education section will outline more about your wellness plan and treatment, such as detailed information about your prescriptions. You also can review the notes on your Medical History.

The link sent via text message is available for 72 hours. You also can download the Care Summaries information as a PDF and email or print it, making it easy to share information with specialists and others, as needed.

Everyone will receive Care Summaries. It is a new federal requirement to provide clinical summaries to all patients in a format they can view, download and transmit.

In addition, Summit Medical Group believes in providing you with timely information and resources that help you take an active role in managing your health and wellness in partnership with your physician.

More patient services are coming soon, including features such as self-scheduling, electronic bill pay and pre-registration, allowing patients to complete and submit forms securely prior to appointments – saving time once you arrive at your doctor’s office.
