Summit physicians recognized for excellence in patient-centered care


The National Committee for Quality Assurance (NCQA) has recognized 38 Summit Medical Group primary care offices for providing excellent care through its Patient-Centered Medical Home (PCMH) program. All 38 Summit practice sites that pursued NCQA PCMH recognition under new 2017 standards succeeded in earning that distinction.

The Patient-Centered Medical Home is a model of care focused on putting patients first and strengthening the relationships between patients and their care teams. It also calls for patients to be active participants in managing their health with a shared goal of staying as healthy as possible, which helps reduce health care costs.

The PCMH concept has been a recurring theme in national health care reform for several years. In 2010, NCQA named 21 Summit Medical Group physicians at four local practices as among the first in the state to receive recognition for its PCMH program.

Currently, several other Summit Medical Group sites are initiating the NCQA Recognition Program process for the first time.
